AFE Dental now includes three plan options: Bronze, Silver, and Gold.
Bronze and Silver list issue ages 18+. Gold lists issue ages 18–69. Eligible family members may also enroll according to plan rules.
The AFE dental product summary lists no waiting period for Bronze, Silver, and Gold. Members should review the certificate for complete details, limitations, and exclusions.
Pricing is based on the client’s resident ZIP code. The 2026 rate guide uses the first three digits of the resident ZIP code to determine the applicable rate area.
Members have flexibility to use in-network or out-of-network dentists. Using an in-network participating dentist may help reduce out-of-pocket costs through negotiated fees.
