Gold Plan

Gold includes a $2,000 calendar-year maximum benefit, preventive care covered at 100%, basic restorative services at 80%, and major restorative coverage at 10% in year one and 50% in year two and later.

Silver Plan

Silver includes a $750 calendar-year maximum benefit, preventive care covered at 100%, basic restorative services at 80%, and no major restorative coverage.

Bronze Plan

How many Dental plans are available?

AFE Dental now includes three plan options: Bronze, Silver, and Gold.

Who is eligible?

Bronze and Silver list issue ages 18+. Gold lists issue ages 18–69. Eligible family members may also enroll according to plan rules.

Are there waiting periods?

The AFE dental product summary lists no waiting period for Bronze, Silver, and Gold. Members should review the certificate for complete details, limitations, and exclusions.

How is pricing determined?

Pricing is based on the client’s resident ZIP code. The 2026 rate guide uses the first three digits of the resident ZIP code to determine the applicable rate area.

Can members use any dentist?

Members have flexibility to use in-network or out-of-network dentists. Using an in-network participating dentist may help reduce out-of-pocket costs through negotiated fees.

Do members need an ID card to schedule an appointment?

Generally, no. The brochure says members do not need an ID card to schedule an appointment, but they will need the primary member’s SSN or Member ID. Georgia and New Hampshire require ID cards by legislation.

Which states are not available?

Availability depends on the selected plan. Silver and Gold are not available in AK, DE, ME, MD, MT, NM, NC, OR, UT, VT, or WA. Bronze is not available in AK, CT, DE, ME, MD, MT, NM, NC, NY, OR, UT, VT, or WA.
